2.2. Registry conduct and assessments
Prescribers participating in the registry must complete the following
forms for each patient:
• A treatment initiation form(TIF)which collects information regarding
indication for use (by checking rCPS, IS, or Other) as well as prior and
current AEDs; in the event a prescriber selects multiple indications, a
representative from SHARE contacts the prescriber and requires that
they designate a primary diagnosis;
• An ophthalmologic assessment form (OAF) which records ophthalmologic
assessments or the reason why a patient is exempted from
testing (e.g., the patient's general neurologic conditions that preclude
the need for vision assessment, patient blindness, co-morbid condition(
s) preventing vision evaluation, or “other” reasons); and
• A treatment maintenance form (TMF) documenting the benefit–risk
assessment, required at 3months if the patient is to continue vigabatrin
