In week 5 of 2014,
several consecutive malfunctions occurred,including a power failure.
This resulted in a jammed acid pump and clogged pipes regarding hydrochloric acid as
well as citric acid. After repairs and clean up actions a manual valve remained open.
This was not mentioned on the work permit and not properly included in the LOTOTO.
Friday the 31th of January, during regular production while applying Hydrochloric Acid
to the reactor V1001, a maximum of 2 liter of acid escaped through the open valve. It
fell from the hose onto the ground. This spill was noticed by the operators who closed
the valve and cleaned up the spill with sufficient water to the sewer system. They
breathed in some of the acid fumes, which resulted in a LTA for one of the operators.
