It is very important to limit contact between immunosuppressed children and persons recently vaccinated with OPV, because the fecal-oral route of transmission can result in paralytic poliovirus infection. Children who are due for immunization and reside in the same household with immunocompromised individuals (e.g., those with HIV or severe combined immune deficiency, persons receiving cancer chemotherapy or prolonged corticosteroid therapy) should receive IPV according to the AAP guidelines and avoid the risk of exposing others to poliovirus.
Follow-up. Children who have paralytic poliovirus require close follow-up care by a multidisciplinary team to prevent further complications.

- Provide immediate referral to a physician for any child suspected of having poliovirus infection.
- Report cases of poliovirus to the health department.
- Contact the school nurse or day-care center.
