1. On completion of the associate's 119 days, a performance appraisal is to be completed by the associate's Supervisor, Manager or Department Head.
2. Human Resources Department will advise all Departments at the end of each month of all associates in their area due for their 119 days probation appraisal in the next month.
3. Blank performance appraisal forms will be sent to the manager or Department Head at the end of each month.
4. The relevant Manager should arrange a suitable time to complete the associate's appraisal, at least 5 days prior to the appraisal taking place, this interview should be conducted without interruptions.
5. The objectives of the appraisal should be made clear to the associate before starting with the appraisal.
6. The Manager should rate the associate on each of the factors noted on the form, explaining the reasons for the rating he/she was given.
7. The Manager must state clearly any areas of an associate's performance that require improvement and provide assistance and training to ensure the associate is given the opportunity to meet the required standards.
8. The probation period may be extended once for no more than 90 days. If an associate's performance is not satisfactory, retraining may be necessary. Whatever the outcome, it must be clearly indicated on the appraisal form and the date for review should also be documented. Areas that attract a `5' rating must provide a full explanation of the previous shortcomings and if necessary a supplementary form attached to the appraisal. Ensure the associate acknowledges the supplementary form by a signature.
9. If probation period is not passed satisfactory and is not extended then termination may occur with prior discussion and approval with the General Manager.
10. The appraisal form and any attachments are to be signed by the relevant Manager, Department Head and Executive Committee Member.
11. The appraisal is then returned to the Human Resources Department.
12. If the appraisal is satisfactory the Department Head can then present the formal confirmation of employment letter to the associate. This letter will have been prepared by Human Resources and held by the Department Head pending a satisfactory appraisal.
13. If the appraisal is not satisfactory, Human Resources will ensure that new review dates are noted and the appraisal is conducted again once the review date has elapsed. The Department Head should consult with Human Resources before the probationary period is complete to discuss any possible assistance or appropriate counseling to the associate.
14. Assist in ensuring any recommended retraining is provided and again that a new review date is set for a new appraisal.
15. The associate's performance appraisal will be maintained on the associate's personal file.
16. Any associates probationary period may only be extend once for 90 days. If still not satisfactory, termination will take place.
