4.5 ADDITIONAL REMARKS TO LOTO PROCEDURESPECIAL WORKS
Special works comprise the works not falling under the category:
• normal operation plus minor interventions and,
• Interventions with complete energy of the machinery (full LOTO);
such as trouble shooting, where functions of the machinery need to be active to find the cause of failures. Another example of special works is commissioning of new machinery, normally carried out by the supplier. It is not allowed to have personnel in the danger zone when not strictly necessary.
A dedicated risk assessment has to be prepared for the high risk tasks In case of special works and work instructions have to be derived from the risk assessment, taking into account the LOTO procedure of the machinery concerned. The risk assessment and the work instructions have to be agreed with the site responsible or project responsible and are part of the Safe Work Permit (see REFERENCES). Only persons with sufficient knowledge and skills are allowed to carry out the high risk tasks.

GROUP OR EXTENDED SHUT-DOWN PROCEDURE
The function of this shutdown is to provide for long-term lock out on the same machine and for conveyance from shift to shift or crew to crew. The procedure also provides safe means for group or individual lock. The location of each lock and tag must be logged.
The primary authorised person takes full responsibility for the Group LOTO procedure. He will apply a multi-lock accepting device on each of the energy-isolating devices and places his lock to it.
All keys of the Safety Locks used in this procedure will be placed in the Group Lock-Out Box. The Lock-Out Box will be locked by each person working on the machine, after assuring its Zero Energy State.
