The obstetrician was contacted and advised of the plan for an angiogram, and instructed for the patient to receive two IV doses of intravenous steroids (to promote lung maturity in the neonate) in the case of premature delivery. Alongside the normal teams in the cardiac catheter laboratory (CCL), the obstetrician and anaesthetic doctor were in attendance throughout the procedure.This is in case there was a need for an emergency section or in the event of the patient requiring intubation and ventilation. Additional lead shielding was put over the abdominal area of the mother to protect the fetus from any radiation.
The use of radiation during angiogram in pregnant patients should be kept to a minimum and is considered safe when appropriate abdominal shielding is used (Salam, 2001). As the patient was having her angiogram in a hospital with no maternity facility, an emergency pack in case of caesarean section was sourced from the theatre department.
The patient underwent a percutaneous coronary angiogram via the brachial approach without complication. The angiogram result showed normal coronary arteries of the left anterior descending artery,right coronary artery, and left main stem. However, there was a 99% occlusion of the left circumflex into which a bare metal stent was inserted. All reported stenting during the acute phase of AMI in pregnancy were performed with bare metal stents. The safety of drug-eluting stents in pregnant women is therefore still unknown.This is because drug- eluting stents require prolonged antiplatelet therapy with clopidogrel, and the incidence of caesarean section deliveries in patient with heart disease is relatively high.The use of drug-eluting stents during pregnancy may be problematic and should be avoided if possible (Roth and Elkayam, 2008).
