The study reveals a 0.8-cm abnormal signal intensity(SI) lesion at left thalamus, showing intermediate SI on T1WI, hyper SI on T2WI/FLAIR, mild restricted diffusion and homogeneous contrast enhancement. Several old lacunar infarctions at right basal ganglia are observed. A 1.2-cm gliotic change with hemosiderin deposit at left basal ganglia is detected.
The ventricular system and basal cisterns are within normal limits. There is no hydrocephalus. Neither shifting of the midline structure nor extra-axial collection is seen.
The calvarium and skull base shows normal marrow SI. The visualized paranasal sinuses, mastoid air cells and orbits appear normal.
MRA of the brain shows mild luminal narrowing of M1 segment of right MCA. There is normal flow related SI and smooth outlines of bilateral ICAs, bilateral ACAs, left MCA, bilateral PCAs, bilateral VAs and basilar artery. Fetal origin of right PCA is noted, normal variation. No abnormal outpouching lesion is depicted.
