The risk screening intervention was perceived by nurses as
valuable in the prevention of pressure ulcers. The actions of nurses
that resulted in the high number of risk assessment tools completed
at admission (and nurses most often choosing the earliest opportunity
to complete the tools) suggest their confidence in this clinical
intervention. Though only 10% of new clients were screened to
be at high or very high risk during the pilot, nurses completed care
plans for 22% of all clients screened, suggesting clients with low
and moderate risk do require structured interventions and close
and rigorous monitoring. A large number of existing clients were
also the target of pressure risk and management care plans during
the pilot period further supporting this suggestion. The client care
plan, designed specifically for pressure risk and management, has
been a valuable addition to the nurse’s suite of clinical tools
