These failure were initial reported to Cameron in May 2014.
We have done some digging on this one with Engineering Manager and my QA manager. This issue actually came up in Q2 2014 on FPR 241051. Summary follows, but I think we can go to the customer and tell them it is a closed issue. We would also welcome doing business direct with PTTEP in the future.
Originally, we were selling these fusible to a GFI, who I believe is a supply house. We were informed by GFI that one of their customers had 9 failures and requested immediate action. We were never provided end customer’s information. Around the same time, we began to see a high failure rate during our internal testing. This plus field failures culminated in a full design review and change, attributed to the fusible plug tip angle. Since change, we have seen 0 internal failures. I will also note that even prior to change, all shipped fusibles passed internal FAT.
To satisfy the customer (GFI), we immediately shipped them 20 replacements at no charge to pass on to their end customer and asked them to return the 9 confirmed failures. We have these 9 fusibles at Tanner, all of which match the 9 SNs listed below, i.e. confirming PTTEP was the customer. Our contacts with GFI through this process were Alan Cardno & Craig Jennings (see 1st attached email chain). It is also noteworthy that GFI has since been supplied 50 additional on SO 2719475 that shipped in late August 2014 (See 2nd attached email chain). We have received no negative feedback or other failure reports.
