Triggers for Acute Exacerbation
Triggering stimuli for suicide most often include a real or anticipated event that causes shame, guilt, despair, or humiliation. In addition, legal problems, financial problems, and feelings of rejection can precipitate suicidal behavior.
Most adolescent suicides are triggered by a stressful event, such as a social problem (often a breakup of a relationship or social isolation), a legal problem, an anticipated loss, family instability, or news of another person's suicide (Cole et al 2011).
Signs and Symptoms
There are no specific physiologic signs and symptoms that define suicide. Assessment of risk factors is important, and all patients should be screened for suicidal ideation. Behavioral warning signs in people at risk for suicidal behavior have been identified using the mnemonic, IS PATH WARM (Gagnon 2007; American Association of Suicidality, Know the Signs, 2013):
I Ideation—expressed verbally or communicated through writing or gestures
S Substance abuse—increased with suicidal ideation
P Purposelessness—lack of reason for living
A Anxiety—agitation, sleeplessness, or sleeping all the time
T Trapped—a feeling that there is no way out
H Hopelessness—having nothing to live for
W Withdrawal—from friends, family, society
A Anger—uncontrolled rage or revenge seeking
R Recklessness—engagement in risky activities
M Mood changes—often dramatic
Potential Complications
The most significant complication surrounding suicide, is patient death.
Acute physiologic complications associated with attempted suicide are specific to the method used to self-harm. The primary complications of attempted suicide include:
Physical trauma from self-inflicted harm with a lethal weapon, including superficial trauma to skin as well as deep-tissue and organ trauma from penetrating injuries
Anoxia from suicide attempted by suffocation, which may lead to permanent neurologic damage
Organ tissue destruction resulting from chemical overdose (Figure 1)

Also associated with suicide and attempted suicide are psychological complications—both for the patient and for the family and friends of the patient. Because previous suicide attempts are a risk factor for future suicide, each attempt presents the potential complication of suicide completion. In addition, especially in the youth population, news of one person's suicide may precipitate another person's suicide. Within the family, a suicide represents a life stressor that may precipitate depression, acting out, or family discordance (Cole et al 2011).
