Atenolol (at an initial dose of 0.5 mg per kilogram of body weight) was increased on the
basis of the hemodynamic response up to a maximum dose of 4.0 mg per kilogram per day
(not to exceed a total dose of 250 mg), with a goal of a 20% or greater decrease in the mean
heart rate as measured on a 24-hour recording.13 Losartan (at an initial dose of 0.4 mg per
kilogram) was adjusted on the basis of body weight up to a maximum dose of 1.4 mg per
kilogram per day (not to exceed a total dose of 100 mg), as recommended by the FDA.13
