Challenges of the Chronic Care Model
In their recent article, Glasgow et al. (2001) state that the changes recommended in the CCM regarding the improvement of the delivery of effective patient care are consistent with those needed to maintain effective prevention of disease and disability. Glasgow and colleagues suggest that both fields follow a common set of system changes and improvement strategies anchored in the CCM. Unfortunately, the paradigm in which these issues are discussed is narrow, evidenced by descriptors, such as "preventive care" and "clinical preventive services." In contrast to the representation of the prevention field offered by Glasgow et al. (2001), effective prevention of illness and disability actually moves beyond the use of clinical services to include community and inter-sectoral collaboration
