Normally an new e-tracker should be raised but email approval is also acceptable provided appropriate approval is obtained .
Without seeing the whole communication I can't judge whether Amex has made any mistake in which circumstance, but clearly it's always employee's responsibility to ensure policy is followed before they contact Amex.
Adding destination for a domestic trip then the domestic approval flow should be followed. Depends on the travel tool (air or car/rail) approval should come from both operation and finance .
If final approval is not obtained then the employee is responsible for the cost since ultimately it is employee's responsibility to comply with company policy .
