N:What seems to be the problem?
Pt:I’m sick!
N:Can you be more specific?
Pt:I’m getting ready to throw up!
N: So you feel nauseated.
Pt:Yes!I’ve been throwing up all night.
N:I see.have you been able to keep any fluids down?
Pt:NO,not even water.
N:Have you seen any blood or “coffee grounds” in your vomit?
Pt:No.Just the last food I ate,and some brownist liquid.
N:That would be bile.It comes from your gall bladder.So when did this start?
Pt:About 2 or 3 days ago.
N:Are there any other symtoms?
Pt:Yes,Fever,headache,and all my muscles and joints hurt.
N : Any other family members with symptoms? Or any friends?
Pt : Yes, my sister. She was sick in the same way last week.
N : I see. Any diarrhea or constipation? Any abdominal pain?
Pt : No.
N : Have you been traveling? Eating out? Eating any new or unusual foods?
Pt : No, I don’t think so.
N : Did you start on any new medications recently?
Pt : No
N : Any history of significant medical problems? Diabetes, heart disease, Kidney or liver
disease?
Pt : No,I’ve been pretty healthy until this.
N : It sounds like a common form of the flu. Did you get your flu shot this year?
Pt : No, I don’t like to get shots.
N : Well, I think you might consider it next time. The vaccine is quite safe for the vast
majority of patients, and it can avoid the most common strains of the flu.
Pt : What can I do for it right now?
N:Unfortunately,it is caused by a virus,and will have to run its course.In sever cases we sometimes have to hospitalize patients,but I think we can manage your symtoms with medication at home. The treatment is pretty mush just symptomatic medication and fluids.
Pt: What about Tamiflu? I heard a bout that during the swine fluepidemic.
N:That only shortens the course of the disease,and it should be started within the first 24-48 hours.i will give you a suppository to control the nausea,and when we can get some fluids into you I will give you some acetaminophen with codeine.
Pt:What’s acetaminophen?
N : It’s the same as Tylenol. It will help with the fever. The codeine will reduce the muscle aches. The symptoms of the flu usually resolve within 5-7 days.
Pt : And antibiotics won’t help?
N : Not if it’s viral.
Pt : Is there anything else this could be?
N : There are several other viral illnesses I’m considering: mono, hepatitis, gastroenteritis. But I don’t see a need to run tests unless additional symptoms develop, or you fail to start improving in the next few days.
