1. The cardholder did not make or authorize other to make this transaction as per the Progressive Cardholder Dispute Letter. (Document 1)
2. The cardholder has never ordered nor received any merchandise or any service from the merchant.
3. The name appeared on the Statement of Account is not the cardholder’s, and she does not know this person.
4. The transaction in question was processed as unsecure Mail/ Phone order/ Recurring transaction. So, the merchant is responsible for the fraudulent transaction.
