Description of the accident:
Employee was returning to the Braiding Room and upon entering the gowning room, slipped and fell.
Root Cause :
As the employee entered the gowning room, the floor was wet after being recently mopped which led to the employees loss of balance.
Immediate action:
It was identified that a wet floor sign was not present in the area. A “Wet Floor” sign was immediately placed in the area following the event.
Retrained housekeeping personnel regarding expectations for placing “Wet Floor” signage in mopped areas.
Action plan:
Revise housekeeping safety procedures to clarify proper placement of “Wet Floor” signs
All housekeeping personnel will be trained on revised safety procedures.
Housekeeping contractor will present options for cleaning floors without leaving behind wet floor or propose waterless floor cleaning methods.
Reinforce employee situational awareness by publishing a Take-2-for- Safety.
