Instructions:

• Create up to five (5) goals in total. Add additional information to complete the goal and any specific targets to be achieved.
• Ensure goals are SMART (Specific, Measurable, Achievable, Relevant, and Time Based).
• Goals can now exceed 100%. Skills Development goals have a weight of 0%
• As part of each employee’s job related development, at least one Skills Development (ex: communication, project management, etc.) goal is recommended.
• Under Results Achieved description, identify if goal was Achieved, Not Achieved, Partially Achieved, or Exceeded
