During the outside grinding operation on the lean cell, the employee injured his hand whilst trying to reset/correct a fallen CVL.(#1) . His index finger was in position (#2) when it was caught between the lining & the feed wheel. At this point the emergency "bump bar"(#3) was activated which stopped the machine. The safety mechanism(#3) & stopper(#4) created the force that created the pinch point between the lining & the rotating drum. As the lining got thicker, the force got greater until it eventually tripped the safety device. The employees finger was crushed which resulted in half of his finger being amputated (1st and 2nd phalanx).
