Sometimes the physician does not consider
weaning, so we have to remind the physician that
weaning readiness needs to be assessed, or that
the patient needs additional intervention(s) to
facilitate weaning. This patient does not have
good nutrition, this patient has excessive
secretions and needs a physical therapy consult
to help with secretion drainage, which could
enhance weaning success. Through coordination
of care, leader nurses are very helpful in preventing
weaning delay or failure and reintubation. (ID,
