For both the third issue and the abovementioned two items, responders
were asked to rate the frequency for each item as follows: 5 ¼ never, 4 ¼
rarely, 3 ¼ sometimes, 2 ¼ usually, or 1 ¼ always. The response for the
two items serves as a key point for further discussion with the clinician if
the responder marked one of the options 1–4. Although the first and third
issues and the two items are rated on the same scale from 1–5, the frequency
rating is worded reversely for the third issue and the two open questions (e.g.,
for the first issue: 1 ¼ never (perform in appropriate phase). For the third
issue and the two open questions, 1 ¼ always (react emotionally as the
result of a lack of success).
