At about 10:45 am, on 23 July 2013, IP (3rd party service engineer) was hit by a sledge hammer resulting in a serious injury to the ridge of his nose.
The task at the time of the incident was removal of crankshaft from the Mud Pump#2 power end housing. A flogging wrench was attached to the crank shaft bolt and was being loosened using a sledge hammer.
When the co-worker swung the 10lbs Sledge hammer, he missed the target (flogging wrench) and the motion of the hammer (still held by the co-work) continued 6 -7 ft in the direction of the IP. This resulted in a glancing blow to the IP’s nose and sustained injury in the process.
IP was given first aid treatment by the IMP on board and later sent ashore for further evaluation. Medical diagnosis confirmed a fracture to the nose.
