2.3.4. Accident scenarios
For the training phase, accident scenarios were developed for each symbol which described an accident or “close-call” and further elaborated on the nature of the hazard, the recommended actions, as well as the possible consequences of failing to perform these actions (see Fig. 2). The accident scenarios were derived from “real-world” reports of accidents from a number of on-line sources (see references) including: the U.S. Department of Labor Occupational Safety and Health Administration Accident Report Fatal Facts, the National Institute for Occupational Safety and Health Alerts, the U.S. Department of Labor Mine Safety and Health Administration Safety Hazard Alerts, and the Centers for Disease Control's Morbidity and Mortality Weekly Report. The scenarios were written in the second person (i.e., using the pronoun “you”) in order to encourage the participants to think of themselves within the depicted situations
