1. Incorrect bolts shipped to site – bolts shipped by Vipco were too long and would not have secured the joints they were supposed to.
2. Short supply of ladder tape- both 100mm and 300mm wide quantities of ladder tape supplied by Vipco were insufficient length to complete erection of the WHRU.
3. Missing tack welds on lining nuts – per the attached photo from site, on inspection of the first upper convection box, a number of lining nuts were found that hadn’t been tack welded onto the studs.
4. Debris in the convection box – at same inspection as above, cut ends of lining studs were left loose in the convection box.
5. Loose header box bolts found – despite appearing to be marked off as tight, a number of bolts on the header box of WHRU 1 upper conbox have been found to be loose, site are now having to check every bolted connection for the same.
