PART 2 – PREPARING FOR THE ASSESSMENT
2.1 SELF-EVALUATION PRIOR TO APPLICATION
A required part of the forensic service provider’s preparation for an assessment is the determination and
documentation by the forensic service provider that it meets all applicable accreditation requirements. To make this
determination, ASCLD/LAB requires several actions prior to application for accreditation.
Any forensic service provider preparing for ASCLD/LAB-International accreditation should be in possession of the
applicable and most current version of the following program documents:
• ISO/IEC 17025:2005 (licensed copy)
• ASCLD/LAB-International Program Overview
• ASCLD/LAB-International Supplemental Requirements (for applicable program)
• ASCLD/LAB-International Conformance File (for applicable program)
• ASCLD/LAB-International Application for Accreditation (with 4 attachments)
• ASCLD/LAB Statement of Qualifications
• ASCLD/LAB Policy on Measurement Uncertainty
• ASCLD/LAB Policy on Measurement Traceability
• ASCLD/LAB Policy on Sampling, Sampling Plans and Sample Selection in the Drug Chemistry Discipline (if
applicable)
• ASCLD/LAB Policy on Use of Business Name, Acronyms, Corporate Logo and Accreditation Symbols
