PLANNING AND IMPLEMENTATION
The fo
llowing nursing interventions are planned and imple-
mented in the intensive care unit.
•Maintain patient-co
ntrolled analgesia.
•A
ssess frequently for pain control and response to analgesia.
•Enc
ourage expression of thoughts, feelings, and fears about
condition and pl
acement in ICU.
•Document
vital signs and heart and lung sounds at least every
4 hours.
•Weigh ever
y 12 hours.
•Document hourly
intake and output.
•Restrict flui
ds as ordered, including diluent for all intravenous
medications as intake.
•A
ssist with mouth care every 3 to 4 hours; allow frequent rins-
ing of mouth and ice chips as allowed.
•A
ssist with position changes at least every 2 hours; teach use of
the overhead trapeze.
•Monitor frequent
ly for signs of infection, bleeding, or respira-
tory distress.
