An individualized plan of care is based on assessment data, Identified risk factors and the Client’s goals. The plan is developed in collaboration with the client, significant others and health care professionals Level of Evidence-IV.
Discussion of Evidence
The risk factors identified in the assessment phase provide the framework for the development of the plan of care. Including the client and family in the development of plan of care is essential for the establishment of mutual goals and adherence to the plan.
