So what really constitutes the 75%+ of human error repeatedly referred to in the
literature? Is human error synonymous with pilot error? To simply ascribe aviation
accidents to pilot error would be a naïve approach to accident investigation. It is well
established that accidents cannot be attributed to a single cause, or in most instances, to
a single individual. The challenge for investigators and analysts is how best to identify
and mitigate the causal sequence of events in that 75% + of accidents associated with
human error.
