Job Summary:
The Staff II Pharmacist practices service-based patient care. As an integral member of the health care team, the Staff II Clinical Pharmacist is responsible for directing and maintaining drug therapy across the continuum of care, providing focused monitoring in the areas of anticoagulation, antimicrobial management and parenteral nutrition therapy.

As an integral member of the health care team, the Staff II Clinical Pharmacist is responsible for directing and maintaining drug therapy across the continuum of care, providing focused monitoring in the areas of anticoagulation, antimicrobial management and parenteral nutrition therapy. The Staff II Clinical pharmacist continuously seeks out interventions by making rounds and offering suggestions on individualized medication regimens. The pharmacist is an integral member of the health care team, also contributing to drug distribution activities, providing drug information, serving as a preceptor to pharmacy students and residents, conducting clinical research, and participating in Department and Hospital Committees, task forces and work groups. They foster excellent interdepartmental relationships and provide quality and cost effective pharmaceutical care to all patients, employees and medical staff of UCDMC. This position works cooperatively with Department administrators, the Medication Safety Team, hospital and medical staff members to design, implement, and monitor quality assurance surrounding medication and patient safety within the health system. Responsibilities may include determining effectiveness of new technology implemented to enhance patient safety, educating staff on appropriate use of new technology, developing and maintaining reports to measure achievement and maintenance of expected outcomes, reviewing medication errors, and reporting results of work to supervisors and hospital-wide safety committees. Essential to this position is the ability to maintain constructive, communicative working relationship with departmental and hospital management and professional staff.--Current California Pharmacist License, Minimum of 2 years of equivalent clinical experience or residency training with emphasis in pharmacy administration and/or medication safety preferred.
--Demonstrated success using Lean Six Sigma quality improvement methodology.
--Computer skills allowing for analysis of large databases, with statistical analysis, and including but not limited to Microsoft Office software.
--Familiarity with the medication use process, available medication use systems technology, and associated risks and benefits of technology. --Familiarity with online incident reporting systems and ability to manage categories relating to medications. Practice sites will include various inpatient hospital areas.
