There was a relatively high response rate (42.7%) with all
professional groups from both tertiary and secondary care
hospitals. The top KPIs were KPI21 (proportion of patients
that has had a chart review conducted by a clinical pharmacist
within 24 hours of admission) and KPI2 (proportion of
patients for whom medication reconciliation was undertaken
and discrepancies resolved). If KPIs were to be classified
into the stages of the medication-management process as
described by Stowasser et al.,
[10]
five of the top 10 KPIs would
be included in the ‘review of medicine order’ stage. The
