Study Design
The purposes of this 2-phase qualitative study using a
content analysis and thematic analysis approach are
to (a) explore the experiences of nurses and nurse
leaders in a 200-bed hospital with a patient care mission
of innovation and (b) generate a thematic map
of the experience. Phase 1 consisted of semistructured
interviews designed to explore experiences of innovativeness
in nursing and determine knowledge and
skills required to create and sustain an environment
for innovativeness. Phase 2 consisted of a focus group
to clarify findings, ensure credibility, and uncover the
constructed reality of innovativeness between nurses
and nurse leaders. Phases 1 and 2 provide the basis for
a beginning conceptual framework depicting interactive
behaviors for innovativeness between nurses and
nurse leaders.
Nature of the Study
Phase 1 of this qualitative descriptive study used an
inductive approach to ensure that the perceptions
were context specific and depended on the interpretation
of specific people in a social context.14 Phase 2
used a constructivist approach to ensure that reality
was reproduced by participants acting on their
interpretations and their knowledge of engaging in
innovativeness.14 Data were analyzed using conventional
content analysis based on the works of Hsieh and
Shannon.15 The selection of content analysis was based
on the need for a conceptual framework. The framework
provided away to listen to thewords of the text
for a better perspective of the produced words.15 In
addition, the realistic thematic analysis method of
Braun and Clarke was used to analyze subresearch
questions. This method was selected because it captured
data in relation to the research question and provided
meaning in the data set.16
Research Questions
The research questionwas ‘‘What is the experience of
nurses and nurse leaders in hospitals that enable
creating and sustaining a climate for innovativeness,
given the current and unique constraints in the world
of nursing?’’ Subresearch questions used to facilitate
the interviews among nurses and nurse leaders were
the following: (a) Describewhat it is like to participate
in generating and implementing new ideas; (b) Describe
a situation where nurses or nurse leaders drove
you to develop a novel idea; and (c) Are there any
forces that can accelerate the acceptance of the new
idea or process?
Participants and Setting
This study was conducted in a 200-bed community
hospital in the southern United States. Recruitment of
potential participants was conducted by contacting all
nurses who participated on hospital committees and
nurse leaders (n = 115) through e-mail after institutional
review board approval was obtained. Potential
participants were invited to contact the researcher if
they were interested in participating in the study. Per
diem nurses, traveling nurses, and nurses not participating
on a committee were excluded. Purposive sampling
resulted in a final sample consisting of 12 nurses.
Six nurseswere in the role of staff nurse and 6were in
the role of nurse leader in the organization. This sample
size is consistent with other studies using semistructured
interviews, which typically consist of 6 to
12 participants.17 The range of employment for participants
in the study was from 1 year to 25 years.
Roles of participants included clinical nurse leader,
assistant director, director, and administrative director
as well as bedside clinical nurses. When nurses
arrived at the study site (conference room), they were
informed of the study purpose and were invited to
participate in the study. Information regarding the
study was provided, and they were asked to sign an
informed consent before the interview began.
