Once we have verified your documents, we will mail your replacement card. You should receive your card within 10 business days from the date on your receipt. Your replacement card will have the same number as your previous card.
Our offices can assist applicants by issuing a notice acknowledging the SSN application while documents are being verified.
You may request the release of your SSN, once it has been assigned, to an interested third party by completing form SSA-3288, Consent for Release of Information or form SSA-132, Notification of a Social Security Number (SSN) to an Employer for Wage Reporting Purposes. Your local office will provide form SSA-132. You may access the SSA-3288 online at:

While the form SSA-3288 is the preferred method for giving consent, if you cannot obtain the form, your written consent must meet all of the following criteria:
- Contain the name, Social Security number, and date of birth of the individual whose information is to be disclosed.
- Be written, signed, and dated by the individual or someone who can consent for him/her.
- Specifically authorize the Social Security Administration (SSA) to release records. As long as SSA is specifically named, our requirement that the consent is addressed specifically to SSA will be satisfied.
- Specify the information to be disclosed.
- State the purpose for which the information is to be disclosed. (This helps SSA determine if a fee is applicable to the disclosure.)
- Specify to whom the record may be disclosed.
You should send or take the completed form or its equivalent to your local Social Security office.

Our 800-number telephones generally are less busy during the early morning from 7 a.m. to 9 a.m. and in the late afternoon and evening from 5 p.m. to 7 p.m. We encourage people to call during these times to avoid busy signals and extended wait times.
