perceived that they had received sufficient information for
only two of those patients, fair information for seven patients
and inaccurate and/or insufficient information for one
patient. The RNs and LPNs reported that they often made
their own assessments, identifying each patient’s individual
needs and how he or she managed eating, since they perceived
that discharge information was not always accurate.
