Today, most insureds in the United States are covered by some type of managed care plan. Managed care is a method of integrating the financing and delivery of health care services within a system that manages the access to health care ser- vices and the cost of those services. A managed care plan is an arrangement that integrates the financing and management of health care with the delivery of health care services to a group of individuals who have enrolled in the plan.
