1. To align with Chevron Fitness For Duty Procedure, contractor companies should develop written job descriptions that describe the essential physical requirements and working conditions of the job. Example of job description with physical requirement is attached.
2. All contract companies/service providers shall provide the health test to determine that their personnel are fit to perform the job on an annual basis.
3. Chevron Thailand has provided the pre-placement / pre-assignment physical checklist that can cover most of the health risks that can endanger the persons if there is no proper control. Some specific positions may require special medical evaluation in order to retain license to work, e.g. crane operator, diver, pilot, confined space worker, etc. For specific positions, please directly consult with Chevron representative.
4. Under Thai Law, the physical examination shall be conducted / reviewed by a qualified occupational medicine physician. The benchmark for the physicians to use as a reference is
attached. This benchmark shall be used to determine fitness for duty of the persons who will be required to work in the offshore environment.
5. The individual detail health check- up document shall be kept at the contractor company. Only the summary report of persons who are fit for duty is required to be submitted to Chevron NST clinic monthly basis starting from 1 May 2014 (directly to STGN@Chevron.com and NSTCLINIC@Chevron.com). The excel format report form is attached. For on-call work, the report should be submitted 7 days in advance of the travel date.
6. When traveling offshore, passengers must carry and show valid health and safety passport to Check-in personnel. Chevron Thailand is also conducting a health screening process prior to the person being allowed to go offshore.
7. Individuals must follow the Return to Work Procedure after all occupational or non-occupational absences. Refer to the attachment for specific information.
8. Individuals are subject to the Chevron Drug and Alcohol Policy (100% breath alcohol test for all passengers and random urine substance test).
