Breastfeeding initiation among NICU-admitted infants increased from 53.3 percent in 2006 to 63.8 percent in 2012 (Cochran–Armitage p < 0.01). Increasing, significant trends in breastfeeding initiation were also observed within each gestational age category over the 7 years (all p< 0.01; Fig. 1). The largest absolute percentage increase was seen among extremely preterm infants (43.8% in 2006 to 70.6% in 2012), resulting in the highest prevalence in 2012. The smallest increase was among late preterm infants (54.0% in 2006 to 61.6% in 2012). Breastfeeding initiation among term infants increased from 54.1 percent in 2006 to 62.7 percent in 2012, among moderate preterm infants increased from 55.2 percent in 2006 to 67.5 percent in 2012, and among severe preterm infants increased from 50.7 percent in 200 6 to 66.6 percent in 2012. As a separate comparison, breastfeeding among NICU-admitted infants did not surpass breastfeeding initiation among infants not admitted to the NICU (95% of which were term infants); however, this gap became increasingly smaller, especially among NICU-admitted moderate,severe, and extreme preterm infants (Fig. 1).
In adjusted analyses, no significant differences in breastfeeding initiation by gestational age among NICU-admitted infants were observed from 2006 to 2010 (Table 2). In 2011, severe (OR 1.6 [95% CI 1.3–2.0]) and extreme preterm infants (OR 1.4 [95% CI 1.0–1.8]) were significantly more likely to initiate breastfeeding than term infants. In 2012, extreme preterm infants (OR 1.4 [95% CI 1.1 – 1.9]) were significantly more likely to initiate breastfeeding than term infants (Table 2)
