The D&C Team continues to deliver high value operations to the BU by delivering quality wells at a very good days per well and excellent completions with incredible perforation footage and economic P&A activities. Because we have such a variety of operations that we deliver to the BU, we also have significant risk exposure in our daily operations. Learning to recognize these risks is becoming more of a challenge each day as we deliver more and more work each day. Several of our recent injuries and high potential dropped objects happened when an individual failed to recognize the risk to themselves or others working around them and did not use SWA to prevent the incident.
As our Swiss Cheese diagram shows, human behavior is the last slice of cheese in preventing an incident. We provide many tools to our individuals and teams to use to assist them realizing and understanding the risks around them and using the correct behaviors to stop the job.
We train in E-Colors so they understand their tendencies on how they could get hurt
We have the IFO pocket book to use to ensure they think about and plan the job properly
The Tenets to support them and help them stop the job
PBED so they don’t become the last slice of cheese
and finally no repercussion for the use of SWA at any time.
All the people working in our D&C operations have the tools needed to develop the right behaviors when they do become the last slice of cheese. As D&C leaders we need to help them understand their responsibility to “stop the job” when necessary to protect themselves, their coworkers and our assets is important to their wellbeing. We also need them to understand that situations occur when they are “The Last Slice of Cheese” and they must be prepared to take action and stop the job when required.
