DISCUSSION
We found significant changes in the outcome of LGA and AGA between the three trimester’s
women, whereas the remaining all GDM associated complications were same for all
trimester’s women. We found no changes in the number of complications between the
groups, the changes in the duration of GDM doesn’t alter the chances for developing
complications. The increase in duration of GDM not increases the number of complication.
Not many studies focused on the duration of GDM by trimester and its effects on pregnancy
outcome. Study by Riskin-Mashiah S et al, 2009,[3] was focused on first trimester glucose
level and its effect on pregnancy outcome, the study found that the elevated fasting glucose
levels in first-trimester increases the risk of adverse pregnancy outcome. Early diagnosis of
GDM significantly has long duration in pregnancy and associate with higher occurrence of
poor maternal and neonatal outcome.
[4, 5] The neonatal complications like LGA, macrosomia
and neonatal hypoglycemia were occur mostly with early diagnosis of GDM,
[2, 6, 7] But study
by Virally M et al, 2007,[8] concluded that with intensive intervention there was no difference
in the pregnancy outcome between early and late diagnosed GDM women. We were found
similar results in our study that there was no significant difference in the outcome of
pregnancy between the groups. We also found more LGA babies with earlier or 1st trimester
diagnosis women but contrary to above mentioned studies we found more babies with
macrosomia and neonatal hypoglycemia in late or 3rd trimester diagnosis women. Our study
recorded third trimester group women had more hypoglycaemic babies following to second
and first trimester group. In our study we found that there was no significant difference in the
development of PIH between the three trimester groups, which is contrary to Jose Barahona
et al study.
