The knowledge representation of EMRs is very important to ensure semantic interoperability and to facilitate reasoning by decision support systems. In this
research, we represent EMR knowledge as an ontology
which focuses mainly on chronic disease management.
Since, there are some differences between the
information model of an EMR and the information
model required for chronic disease management, the
W3C proposed Computer-Based Patient Record (CPR)
ontology [7] is customized and mapped onto the WHIC
proposed chronic disease management model [2]. Our
proposed ontology ensures a structured means for data
entry by integrating controlled vocabulary from
SNOMED-CT. Furthermore, the resultant ontology is
mapped to HL7 RIM to capture the clinical messages
written in HL7.
