Dear all, as per meeting today we found many serious major defects from last week without root cause analysis.
Dear K. Ball, please make sure to check everyday production defect from the day before and discuss about any major case or repeated case.
Dear team please feedback to me this Friday further root cause analysis about:
Major production process defects:
- Wrong terminal used (one wire found): operator used terminal fast-on from another product. Another operator assembly and test ok, found by QC visual inspection:
From where can come the mix? Check in stock, check previous MO, audit in line no remaining raw material from previous MO
- Use wrong screw on electrovolve (1 piece, screw normally by kit with electrovolve). Cannot find for what purpose we have wrong screw used.
Please confirm from where is the wrong screw?
- Wrong connector orientation, cannot test pass. Possibly operator didn’t finish connector assembly during test.
Please confirm and inform name of operator. Review process assembly details and instruction steps assembly and test.
- 6 pieces forgot torque cable gland switch.
Process screw to update on faiveley line
- 2 pieces terminal not locked in connector.
What action improvement? Tool insertion? Blue press? Jig Spring? Jig probe?
Other problems process development:
- Connector polarization not tested: polarization is assembled on connector after connector assembly (separate piece from connector cap).
Add case in NPI check list.
Verify on JIG why missing and how polarization has been verified and approved?
- NPI control plan check list not reviewed in case of customer design modification.
Need to add review of impact on NPI when receiving revision update in engineering W-I.
