The home care phase could not be meaningfully described
as a process except on a normative level. A large number of exceptions disturb the regular sequence, as illustrated in Figure 3. A patient’s medical condition could deteriorate unexpectedly, problems with performing the HHD treatment may occur, or some other illness could complicate the situation. In such situations a patient may call for help, and/or be taken to outpatient or inpatient care. If the problem can be solved, the patient can resume HHD; if not, the care form may need to be revised.
