The Committee on Administrative Practice of the American Public Health Association appointed, in 1942, a Sub-Committee to make recommendations concerning appropriate units for local health work, and this report by the Chairman, Dr. Haven Emerson, is the result. It is unique in the history of medical service surveys for in this a task has been attempted which is a consideration of the existing public health facilities afforded for the health of the United States, State by State and county by county. After consideration of this huge mass of information, the Committee has made suggestions for a reorganization in each State, mostly by grouping the counties, whereby administrative areas could be established, which would be economical in finance and in personnel, yet sufficiently large to provide agreed minimal standards of service to the public. The proposed regroupings within the States were first submitted to the health officers of the States concerned, and modifications were made as a result of their critic.
When the last census was taken, in 1940, there were 3, 070 counties with an average population of approximately 42, 000, and 27 cities independent of county organization (in four of these the city and county areas were co-terminous). It appeared to the Committee on completion of their survey that approximately 40, 000, 000 of the people of the continental United States " are living in communities where local health services have either not been undertaken at all or are under the direction of part-time and generally untrained or inexperienced health officers. . . ". The Committee considered that the. usual minimum population of an administrative area for health purposes should not be less than 50, 000, and they have made proposals which are set out in detail for each State, for a reduction in number of the local government health areas to 1, 197. It is estimated that an income of between one and two and a half dollars per head of the population will be required if the basic standards deemed necessary by the Association are to be attained.
The industry with which this inquiry has been pursued must have been prodigious, but its direction has shown intelligence and single-mindedness comparable to that industry, and the result is an achievement reflecting the highest credit on the Association and its members. Page by page, table by table, the services in each State are remorselessly analysed until the weaknesses are laid bare and then the remedies suggested seem inevitable. This report should be read by all concerned with the provision of health services and should be in the library of every large health department and in all professional libraries.
