Complaint acknowledgement was sent to the customer.
Immediately received 2 drums of CO10 back and send the replacement on June 11, 2015.
Investigation:
GSH of Local customer is kept at Warehouse operator, and they manage stock and use it by referring from Finished Goods and Shipping Form.
Warehouse operator ordered and issued wrong GHS for CO10.
During checking final package before shipping, the operator checked only have GHS on the drum but did not look into the label that it is correctly or not.
Root cause analysis:.
GHS label was issued by Warehouse Operator, and brought the wrong label to the drum.
Product integrity check, Warehouse Operator did not look into the detail of GHS label that it is correctly as product or not. They just checked that have GHS label
