With regards to LSP-0004 Minutes of Meeting, it is actually a record form as well (You can see this in the Header).
I hope you do not get confused…
The rationale is that "Minutes of Meeting" can be available in many other forms such as emails or other kind of means. As a management system needs to have support on decisions and actions after important events, it is a proposal to use a Standard Practice document as the one developed.
This leaves the room to the organization to use also those other means, if it was a form, all the organization should have to use the form.
I generally use standard practices type of documents to allow flexibility during External Audits - this to prevent the auditor to ask for decision and actions not captured by the management system.
Do you see the trick ? (Standard practices are considered as not auditable)
