DOCUMENTATION DEVELOPMENT AND DISTRUBUTION
A Document Content and Structure
The structure and format of SSHE documents prepared by each PTTEP function may vary in
appearance and writing style, according to its specific needs, but the identification and management of
the documents should be consistent in approach and content. Asset with the existing document
management system may continue to use it. However, future update system should follow this
requirement.
However, for the new operating assets, the structure/document coding/format/appearance of SSHE
documents shall be identical and consistent in approach and content as per required in this SSHE
Documentation Management Standard.
SSHE documents shall follow the document structure as indicated below:
a. Cover page
This section shall contain at least the document title, hierarchy, coding and revision number.
b. Approval Register
This section shall define the author, document owner and custodian and contain signatures of the
technical review authorities.
c. Revision History

This section shall contain the revision history of the document, stating, at a minimum, version
control log and changes from previous version.
d. Document Approval
This section shall contain signatures of all defined relevant approval authorities.
e. Section 1 - PURPOSE
The section shall provide a short description of the objective of the document, a summary as to
why the document is necessary, and results expected from the document.
f. Section 2 - SCOPE
The section shall describe whom the document applies to and define the breadth and limitations
of the document.
g. Section 3 - REFERENCE
This section shall contain a list of references as used in the document.
h. Section 4 - DEFINITIONS (If applicable)
The section shall contain words, expressions and abbreviations used in the document that need
to be defined more precisely.
i. Section 5 - RESPONSIBILITIES
The section shall describe the specific responsibilities resting on and the authority invested in
separate role.
j. Section 6 - REQUIREMENTS
The section shall contain sufficient content as to how the objective is to be achieved.
k. Section 7 - APPENDIX (If applicable)
This section shall provide additional information as required.
To ensure the consistency of the SSHE documents, SSHE document template as shown in Appendix 1
shall be used. The author has the responsibility to create any SSHE document using the template allocated.
