Participants were recruited from an outpatient, primary care clinic at an urban hospital
in the northeast United States. Eligibility criteria included self-identified Puerto Rican
ethnicity, age 18 years or older, and a diabetes diagnosis of type 2 for >1 year. Clinic
staff members identified and contacted eligible patients by phone. Of the 129 patients
who were scheduled, 118 arrived at the clinic to participate in the study.
