6.2.2.2 For any employee in Grades 6 - 10, his/her family will only be entitled to paid specialist treatment (outpatient) after the employee has successfully completed his/her probation period.
6.2.2.3 Before seeking specialist treatment (outpatient), the employee will need to obtain approval from the General Manager or Managing Director.
6.2.2.4 If any employee should exceed the applicable limit, he/she can make a special representation to the General Manager or Managing Director for the Company to pay the exceeded costs. Approval shall be considered on a case-by-case basis and granted at his discretion.
6.2.3 Hospitalization
The hospitalization plan is designed to assist employees to cope with the financial burden
resulting from illness that requires surgery or hospitalization. To that end, health
insurance providing adequate cover is taken out by the company.
6.2.3.1 All Singaporean employees will be included in the company’s insurance plan after successful completion of their probation period.
6.2.3.2 Coverage and limits apply as defined in the respective insurance policy
6.2.3.3 If any employee should require treatment exceeding the applicable limit, he/she can make a special representation to the General Manager to have the costs exceeding the limit borne. Approval shall be considered on a case-by-case basis and may be granted strictly at Company’s discretion.
6.2.4 Maternity Cost
Maternity benefits will be provided to any confirmed female employee to a maximum limit of S$ 1,000 per pregnancy per year.
6.3 Types of Medical Treatment Excluded
The following types of outpatient treatment are excluded from coverage by the Company:
• Dental (except for Grade 6 to 11);
• Cosmetic treatment for beautification purposes;
• Complications arising from pregnancy, infertility or birth control treatments;
• Miscarriage or abortion
• Self-inflicted injuries, injuries sustained while involved in unlawful acts or provoked assaults or any injuries suffered by participating in riots and unlawful assemblies;
• Psychiatric treatment;
• Injuries resulting from suicide attempts or social diseases;
• Outpatient physiotherapy treatment;
• Eye glasses, contact lens, special braces, appliances or equipment;
• Drug addiction, alcoholism and AIDS;
• Treatment for obesity, weight reduction or improvement.
6.4 Medical Claims
Employees are required to submit original receipt for medical expenses incurred.
7. STAFF BENEFITS
7.1 Transport Allowance for Overtime Work
7.1.1 Transport Allowance
7.1.1. (1) All employees are entitled to claim taxi fare if they work in office till after 8.00pm.
7.1.1. (2) All employees required to work on weekends and public holidays for 4 hours or more are entitled to claim taxi fare.
7.2 Transport Claims
Employees are required to submit original receipts for their Transport Claim. Transportation costs incurred for business purposes shall be reimbursed as follows :
