When nasal congestion is the chief

When nasal congestion is the chief complaint, clinical
guidelines for AR recommend use of an oral antihistamine
plus an oral or topical decongestant, or
monotherapy with an intranasal corticosteroid
(ICS).7–9 Oral antihistamines, decongestants, and ICSs
all relieve congestion.45 Because first-generation antihistamines
and older second-generation antihistamines
did not generally exhibit efficacy in the treatment of
nasal congestion,46–51 early studies in AR excluded this
symptom as an outcome measure.52–54 Thus, there are
limited data on the efficacy of older second-generation
antihistamines for nasal obstruction.55,56 More recently,
however, clinical data have emerged suggesting
that the newer antihistamines desloratadine, fexofenadine,
and levocetirizine may be capable of relieving
this bothersome symptom.35,39,51
