Original Australian Federal Police clearance – complete disclosure.

If you have lived in Australia for a total of 12 months or more in the last 10 years (these 12 months are calculated cumulatively, not consecutively) you need to obtain an Australian Federal Police (AFP) clearance by completing a National Police Check Application Form. The form is available from the AFP website at www.afp.gov.au/~/media/afp/pdf/n/national-police-check-application-form.ashx. Please send your completed form to the AFP at the address on the form. If you do not have access to the internet you can request for a form to be posted or faxed to you by calling the AFP helpdesk on +61 2 6202 3333.

When completing the National Police Check Application Form please use Code 33 at Question 1 on the form and include details of any, and all, names you have been known by. If an AFP clearance is provided based on incorrect information, the department may request another clearance. Please note finger prints are not required.
You must respond to this request for additional information within 28 days after you are taken to have received it. As this request was sent to you by email, you are taken to have received it on the date this email was sent. If you do not reply within this timeframe, your application may be decided without the requested information being taken into account. If you are unable to provide this information within this timeframe, please provide the reasons within this timeframe as soon as possible.
Please present this letter to our Visa application Centre (VFS) when you submit additional documents.
