coordinator of care
the nurse acts as a coordinator of care to assure the appropriate sequence of events in the patient's plan of care. leadership skills are required to coordinate the plan of care and to refer patients to appropriate sources when indicted. in the role of coordinator of care, the nurse also functions as a facilitator by directing care within the health care system to meet patient needs and to prevent the duplication of services. As the coordinator of care, the nurse is expected to communicate and collaborate with members of the interdisciplinary health care team to promote and maintain the optimal health status of patients and their families (TBON, 2010).
