Process tubing fitting failures - A look at what happened Chuandongbei Gas Project • Chuandongbei (CDB) Operations started to flow sour gas from a well when a Parker tubing fitting on a chemical injection pump used to pump Monoethylene Glycol into the wellhead failed. • There was a resulting Loss of Containment of 18% H2S gas to the atmosphere and the well was immediately shut-in • No injuries or impact to the surrounding community were reported. • A subsequent investigation identified the following root causes for the tubing failure: inadequate field installation practices, use of improper materials, and unbalanced stress on the threaded connection.
